Job Summary
MDS Coordinator (LPN) responsible for completing MDS assessments and CAAs, compiling data and preparing statistical reports from health records, and personalizing assessments to evaluate quality of care and utilization for clinical reimbursement. Coordinates with physicians, nurses, and health personnel to ensure accurate medical records. Must hold an active LPN license (or Nurse Compact License) and participate in the facility's leadership and on-call teams. This full-time role offers comprehensive benefits and requires every 3rd weekend work and on-call rotation; familiarity with MDS processes, CAAs, and Point Click Care (PCC) is beneficial. Required Qualifications
- Current State of Iowa Licensed Practical Nursing (LPN) license, or an active Nurse Compact License (NCL)
- MDS assessments experience
- Care Area Assessment Summaries (CAAs) experience
- Knowledge of Point Click Care (PCC) preferred
- Ability to participate in on-call rotation
- Ability to perform Essential Functions with or without accommodation
