MDS COORDINATOR

Job Summary
Coordinate and oversee the MDS process for residents, including triggering due dates, coordinating completion by all disciplines, reviewing data for accuracy, inputting MDS into the system, ensuring timely transmission/approval by the State, correcting rejected records, completing CAAs per state guidelines, and formulating the Resident Care Plan (RCP) with the interdisciplinary team. Schedule and conduct RCP meetings, communicate the RCP to staff, monitor compliance, and collaborate with in-house and ancillary departments to ensure adherence to the RCP. Requires RN license and experience with MDS-RAP, Care Planning, and preferably MDS 3.0 and RUG-IV. Performs other related duties as directed by DON/Administrator.
Required Qualifications
- Registered Nurse (RN) license
- current RN license from the North Carolina Board of Nursing
- experience with MDS-RAP and Care Planning functions
- prefer experience with RUG-IV
- prefer experience with MDS 3.0
