Job Summary
Coordinate the Minimum Data Set assessment process for all residents, conducting comprehensive pre-assessments and utilizing standardized tools to gather data on physical, psychological, and psychosocial status. Collaborate with nurses, therapists, and social workers to develop individualized care plans, while ensuring documentation accuracy and compliance with federal and state regulations. Manage assessment scheduling for new admissions, quarterly reviews, and discharges, utilizing MDS software to enter data, generate reports, and maintain resident records. Monitor resident conditions to update assessments and support billing accuracy for Medicare and Medicaid. Provide staff training on assessment protocols and participate in care conferences to discuss resident progress. Full-time weekday schedule with rotating on-call and weekend duties.
Required Qualifications
- Valid license as a Registered Nurse or Licensed Practical Nurse (RN/LPN) in the state where work is performed
- Minimum of 1-3 years experience working as an RN in a healthcare setting
- Strong clinical assessment skills
- knowledge of nursing principles, procedures, and best practices in long-term care
- Proficiency in MDS software and electronic health record systems
- Excellent organizational and time management skills
- ability to prioritize tasks and meet deadlines in a fast-paced environment
- Understanding of federal and state regulations governing MDS assessments, reimbursement system, and skilled nursing facility operations
- Excellent communication and interpersonal skill
- ability to collaborate effectively with team members, residents and their families

- Ability to build rapport with patients through a compassionate and caring demeanor
- Physical Requirements: Work performed in a medical setting due to the need to work in person with residents
- Must be able to sit, stand, bend, reach, talk, hear, stoop, kneel, crouch, stretch, and twist
- Manual dexterity and hand-eye coordination for using computers, entering data into MDS software, and completing documentation
- Stamina to stand and move about the facility to perform frequent rounds and conduct resident assessments
- Emotional resilience and ability to cope with the demands of coordinating MDS assessments and ensuring compliance with regulator reequipments
- Visual acuity and auditory ability to assess residents' condition, read medical records, and communicate effectively with staff and residents
- Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager
