MDS Coordinator

Job Summary
Oversee accurate completion of MDS, Care Area Assessments, and Care Plans in accordance with federal and state regulations. Act as an in-house Case Manager to develop appropriate care pathways, communicate with residents, families, and third-party payers, and ensure timely electronic submission of state database reports. Proactively identify regulatory risks and clinical trends by analyzing QI/QM data with IDT members, while facilitating discharge barriers and educating staff on the RAI process. Manage Medicare and Medicaid coverage schedules, start coverage for newly qualified patients, and determine necessary reimbursement documents. Requires proficiency in MDS 3.0, active RN or LPN license, and minimum two years of LTC clinical experience.
Required Qualifications
- Proficiency in MDS 3.0
- Demonstrating knowledge of state and federal regulations
- Registered Nurse or LPN with current, active license in the state
- Minimum two years of clinical experience in LTC setting
- Ability to stand for long periods of time
- Ability to walk throughout facility
- Ability to bend, kneeling, stooping all day
- Ability to frequently lift or move objects weighing over 20 pounds
