Document Minimum Data Set (MDS) assessments and care plans for residents, ensuring compliance with State and Federal Regulations. Maintain accurate electronic health records and coordinate with nursing staff to track resident status changes. Requires a current NYS RN license and 2-5 years of Long Term Care experience.
