Job Summary
MDS Coordinator role covering Bloomer facility and Regional Vent Center. Responsibilities include coordinating MDS/RAI/PPS activity, overseeing Medicare/insurance/VA programs, maintaining OBRA/PPS schedules and electronic records per state and federal rules, and ensuring accurate medical record documentation to support the RAI process. Requires Wisconsin RN licensure and a nursing degree; preferred 1 year MDS experience and familiarity with PDPM, utilization review, Medicare/VA benefits, and long-term care basics. Required Qualifications
- one-year MDS experience preferred
- current Wisconsin licensure as a registered nurse
- degree from an accredited school of nursing
