Job Summary
Coordinate and complete MDS assessments in accordance with state and federal regulations, ensuring documentation and submissions are accurate, complete, and timely. Collaborate with nursing, therapy, social services, dietary, activities, and leadership teams to support resident care planning, while monitoring documentation for quality, compliance, and reimbursement. Review Medicare and Medicaid requirements, monitor case mix index, and identify opportunities for improvement. Provide education and guidance to clinical teams on MDS processes and documentation standards, partnering with leaders to address gaps and support regulatory readiness through audits and surveys. Maintain strong knowledge of MDS, RAI, and skilled nursing documentation expectations. Full-time role requiring active RN license in Nebraska or compact state, previous MDS experience, and RAC-CT certification preferred.
Required Qualifications
- Active RN license in Nebraska or a compact state
- Previous MDS experience
- Knowledge of Medicare/Medicaid reimbursement and state/federal regulations
- Ability to speak, read, and write English
- Ability to independently lift up to 35 pounds
- Ability to perform repetitive sitting, bending, standing, walking, and movement as needed
- Ability to work in a fast-paced environment with strong attention to detail
- Ability to utilize technology, clinical systems, and documentation resources efficiently and effectively
- Ability to manage deadlines, regulatory requirements, and competing priorities
Desired Qualifications
- RAC-CT certification
- Skilled nursing facility or long-term care experience
- Strong attention to detail with documentation, assessments, care plans, and submission timelines
- Excellent communication skills with nursing, therapy, leadership, and interdisciplinary team members
- Ability to identify documentation gaps and follow up with the appropriate team members
- Strong organization skills with the ability to manage deadlines and competing priorities
