Job Summary
MDS Coordinator role in a Skilled Nursing facility focused on care quality and regulatory compliance. You will develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals, perform resident assessments, and assist in discharge planning. The position requires applying nursing knowledge to coordinate interdisciplinary care, ensure residents’ safety and comfort, and support the team with timely documentation and care coordination. Strong attention to detail, clinical documentation, and collaboration with CNAs, RNs, and other staff are essential, along with commitment to resident-centered care and ongoing quality improvement. Required Qualifications
- Unencumbered State of Iowa RN License
- Graduate of an accredited nursing program
- CPR Certification
- Prior experience as an MDS Coordinator
- 1 year of RN experience in a Skilled Nursing Facility setting
- 1 year of CNA experience (preferable)
