Job Summary
Coordinate the collection and submission of accurate and complete Minimum Data Sets (MDS) in accordance with federal regulations and company policies. Conduct detailed assessments of resident needs and preferences, including social, emotional, and environmental factors, while maintaining up-to-date records and files. Develop relationships with residents, families, and the interdisciplinary team to ensure comprehensive understanding of resident needs. Collaborate on care plans, monitor MDS data for trends, and contribute to quality improvement initiatives and policy development. Requires minimum one year of healthcare experience with gerontology focus, proficiency in MDS software, and strong communication skills. Required Qualifications
