Job Summary
MDS Coordinator role performing MDS/RAI and assessments using medical records, ensuring accurate transfer of data to MDS documents; monitor resident condition changes for MDS relevance; maintain MDS/RAI to optimize reimbursement; complete all necessary documentation in a timely manner; RN incumbents may perform clinical-management duties; requires RN or LPN licensure with long-term care experience, strong communication and organizational skills, and knowledge of skilled nursing regulations. Position is on-site in Edgerton, WI with full-time schedule (Mon-Fri days, some weekends/holidays).
Required Qualifications
- Current RN or LPN license in Wisconsin or eligible for Wisconsin licensure by reciprocity
- Long-term care or skilled nursing experience
- Knowledge of current laws and regulations regarding skilled nursing and long-term care
- English literate with excellent communication, organizational, and problem-solving skills
- Ability to meet physical and job-related requirements
