Job Summary
Oversee the MDS process in a skilled nursing facility, coordinate all MDS assessments in compliance with federal and state regulations, develop and maintain individualized care plans based on assessment outcomes, collaborate with the interdisciplinary team to ensure continuity and quality of care, monitor PDPM requirements and ensure documentation supports accurate Medicare reimbursement, review Quality Indicator (QI) and Quality Measure (QM) reports and implement corrective actions, participate in state surveys/audits/regulatory inspections, ensure timely scheduling, completion, and electronic submission of MDS assessments, educate and support clinical staff regarding MDS, PDPM, and regulatory updates, and maintain accurate, organized, and compliant documentation practices.
Required Qualifications
- Active Florida RN or LPN license required.
