Job Summary
Conduct comprehensive assessments of residents' medical, functional, and psychosocial status to ensure accurate and timely completion of initial, quarterly, and annual MDS assessments. Collaborate with interdisciplinary team members to develop individualized care plans and monitor changes in residents' conditions, updating plans as necessary. Serve as a resource to staff regarding MDS documentation and compliance requirements while participating in quality improvement initiatives. Maintain up-to-date knowledge of regulatory guidelines and best practices in long-term care. Requires current RN or LPN license in Illinois and strong communication skills.
Required Qualifications
- Current RN or LPN license in the state of Illinois
- Strong understanding of MDS assessment process and regulations
- Excellent communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Detail-oriented with strong organizational skills
- Compassionate and patient-centered approach to care
