MDS Coordinator

Coordinate and document assessments of residents and assist in developing comprehensive care plans aligned with facility, state, and federal guidelines. Monitor residents to ensure individual care plans are followed and updated as necessary. This role requires a registered or licensed nurse in good standing with at least one year of MDS experience, located at Providence Park Rehabilitation & HealthCare Center in Mobile, Alabama.
