Job Summary
Coordinate the Minimum Data Set (MDS) process, including conducting resident assessments, reviewing assessments, and submitting MDS data to regulatory agencies. Ensure accurate and timely completion of MDS assessments, care plans, and progress notes while collaborating with the interdisciplinary team to develop and implement individualized care plans. Maintain accurate and up-to-date records and documentation, participate in quality improvement initiatives, and provide education and training to residents, families, and staff on MDS processes and regulations.
