MDS Coordinator

Conduct and coordinate the development and completion of resident assessments (MDS) and care plans, including quarterly and annual reviews. Maintain and periodically update written policies and procedures that implement MDS and care plans. Assist the resident in completing the care plan portion of the discharge plan and help Facility directors and supervisors schedule resident assessment and care plan meetings. Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of newly admitted residents. Assist in determining appropriate treatment, selecting activities, and exercises based on the medical and social history of residents. Requires Indiana RN license, MDS 3.0 knowledge, and long-term care experience. Salary $70K-$85K based on experience.
