MDS Coordinator

Job Summary
Coordinate and complete Minimum Data Set (MDS) assessments, review medical records to validate data, and submit reports to regulatory agencies within required deadlines. Develop individualized care plans, facilitate interdisciplinary meetings, and communicate findings to nursing staff, physicians, and therapists. Maintain compliance with Centers for Medicare & Medicaid Services guidelines, CMS regulations, and state requirements by participating in surveys, audits, and quality assurance activities. Support reimbursement processes through accurate coding and RUG/PDPM classification while collaborating with billing departments to maximize revenue. Analyze quality indicators, monitor resident outcomes, and assist with infection control and risk management initiatives. Requires an active RN license, 1–3 years of long-term care experience, and proficiency in electronic health records.
Required Qualifications
- Active Registered Nurse (RN) license
- Minimum 1–3 years of experience in long-term care, skilled nursing, or rehabilitation
- Strong knowledge of MDS 3.0, RAI process, CMS regulations, and PDPM
- Proficiency in electronic health records (EHR) systems
- Excellent organizational, analytical, and communication skills
- Clinical assessment and documentation

- Ability to sit, stand, walk, and use a computer for extended periods

Desired Qualifications
