MDS Care Plan Coordinator

Coordinate and complete the MDS assessment process in accordance with state and federal regulations, ensuring accurate and timely documentation. Develop and maintain individualized resident care plans with the interdisciplinary team while monitoring assessment schedules and regulatory deadlines. Review clinical documentation to support quality outcomes and appropriate reimbursement, and collaborate with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care. Assist with survey readiness and maintain compliance with facility policies and procedures. This role supports the clinical leadership team at Rock Creek Rehabilitation & Healthcare Center in delivering exceptional resident-centered care.
