Job Summary
Assist the MDS Coordinator with developing and maintaining accurate individualized care plans with measurable goals using the designated format for long-term admissions. Ensure acute PCP is completed within 24 hours and maintain up-to-date care plans for all residents. Conduct MDS assessments using the designated format, advise and communicate the plan of care to nursing staff, coordinate care plan conference schedules, and stay current with state and federal laws governing the care plan process. Must interact courteously with managers, co-workers, residents, vendors or associates to preserve productivity and morale. Role requires an Associates or Bachelor's degree in Nursing and licensure in NC, with experience in LTC/SNF. PRN/part-time scheduling with comprehensive benefits.
