Job Summary
Complete accurate and timely MDS assessments for residents, reviewing clinical records and ensuring documentation reflects individual conditions and care levels. Coordinate with nursing, therapy, social services, and dietary departments to support care planning, while participating in interdisciplinary meetings. Monitor all assessments for accuracy, consistency, and compliance with CMS, New York State, and federal regulations to ensure proper reimbursement under the Patient-Driven Payment Model (PDPM). Maintain strict adherence to MDS 3.0 standards.
Required Qualifications
- Active New York State RN license
- Previous MDS experience in a skilled nursing or long-term-care facility
- Strong understanding of MDS 3.0, PDPM, and care planning
- Excellent organizational and documentation skills
- Ability to meet deadlines and work collaboratively with an interdisciplinary team
