MDS Assessor needed in Brooklyn, NY to perform accurate MDS 3.0 assessments, update and distribute monthly care plan calendars, maintain and audit clinical records, and communicate issues to the DON while protecting resident confidentiality. Requires NYRN license, MDS 3.0 experience, and long-term care experience; must be highly organized with solid computer and communication skills and be a collaborative team member.
