Maternal and Pediatric Nurse Home Visitor

Job Summary
The Maternal and Pediatric Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for Strong Families Vermont (SFVt) services by conducting home visits and adhering to the Maternal Early Childhood Sustained Home (MECSH) model. Duties include maintaining client caseload, participating in community coalition activities, and collaborating with professionals to support client needs. Candidates must hold a Bachelor’s degree in nursing and be licensed as a Registered Nurse in VT and NH, with two years of relevant experience preferred. Strong communication skills and basic computer proficiency are essential.
Required Qualifications
- Bachelor’s degree in nursing from an accredited college or university
- Registered Nurse - licensed in VT and NH
- Current CPR required
- Excellent written and verbal communication skills
- Basic computer skills
- Working knowledge of medical terminology, electronic medical record preferred
Desired Qualifications
- Two (2) years’ experience in maternal/child health, public health, home visiting or mental/behavioral nursing
- Experience in home health care setting
