MAT Registered Nurse/Care Manager

Act as the primary contact champion for the MLK-Wyandanch site location, completing MAT intakes by phone or in person and drafting discharge paperwork. Manage patient care plans, recovery support plans, and panel monitoring via Excel spreadsheets while triaging presenting issues and providing basic nursing skills at every visit. Collect urine drug screens and pregnancy tests, review labs, and handle prior authorizations to coordinate care with external agencies. Provide opioid overdose prevention education to staff and patients, attend weekly case conferencing, and liaise with pharmacy for scheduling and rescheduling appointments. Serve as a patient advocate and crisis manager within a multidisciplinary team focused on harm reduction approaches.
