Job Summary
MAT Care Coordinator coordinates medication-assisted treatment care with medical and behavioral health teams, serves as the primary MAT services contact, conducts intake for substance use treatment, maintains timely documentation in the EHR, performs pre-visit planning, manages patient referrals and coordination of internal/external services (housing, counseling, crisis intervention, financial assistance), schedules transportation/gas cards as available, ensures compliance with MAT program guidelines, collaborates with peers and care teams, and participates in quality improvement initiatives.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Health and Business Administration or other human service field preferred
