Job Summary
Provide clinical social work services, coordinate and complete discharge planning with an interdisciplinary team, ensure compliance with government and accrediting standards, interface with facility departments and outside agencies to develop a complete plan of care, and serve as liaison between families and the interdisciplinary team. MSW degree required and responsibilities include documentation in accordance with facility policy and procedure and resource coordination for patient care in a healthcare setting.
