Job Summary
CSWA Social Worker provides biopsychosocial/spiritual assessments, collaborates with patients and support systems to develop and implement evidence-informed person-centered care plans across the care continuum with the Inter/Multi-Disciplinary Team (IDT), and adheres to NASW ethics and Standards of Practice. The role supports weekend coverage on a Per Diem/On-Call Day shift at Providence Newberg Medical Center in Newberg, OR, requiring a day-shift presence with some weekend commitments and supervision guidance.
Required Qualifications
- Master's Degree in Social Work
- Oregon Clinical Social Work Associate License within 60 days of hire
