Job Summary
Provide psychosocial assessments of patients related to illness and environment, communicate findings to the registered nurse, perform social evaluations, and plan interventions. Maintain clinical records for patients referred to social work, provide information and referral services to patients and families, and serve as liaison between patients/families and community agencies. Develop and sustain contracts with public/private agencies, collaborate with the sales team to increase community involvement and promote services, participate in discharge planning, and support total care plans and case conferences. Maintain collaborative relationships with staff, assist physicians and team members in understanding social and emotional factors affecting health problems, and perform other duties as assigned.
Required Qualifications
- Bachelor’s or master’s degree in social work from a state accredited program
- Minimum of one year’s experience in health care
- CPR certification
- Must be a licensed driver with an automobile that is insured
- Capable of performing the job functions with or without accommodations
