Master Social Worker - MSW Part time

Master Social Worker (MSW) position, entry level, serving patients treated with dialysis with psychosocial assessment, care planning, counseling, and education. Provides psychosocial support to patients and families, collaborates with the interdisciplinary healthcare team, assists with transplant referral processes, facilitates access to community resources, and supports quality improvement activities (QAI) to enhance patient quality of life. Requires a Master of Social Work and possession of a state license; role includes patient education, adherence support, and participation in team-based care for dialysis patients. Travel to facilities as needed is described in duties.
