Job Summary
Provides psychosocial services to dialysis patients, conducts assessments and care planning, and delivers counseling to support rehabilitation and quality of life. Collaborates with the interdisciplinary team to address emotional, psychological, and behavioral impacts of kidney disease, facilitates transplant referrals, educates patients and families, assists with advance directives, coordinates referrals to community resources and insurance/financial assistance, and contributes to quality improvement and documentation as part of the care team.
