Job Summary
Master Social Worker (MSW) responsible for psychosocial assessment, care planning and counseling of dialysis patients, education of patients and families, referral to community resources, and coordination with the interdisciplinary team to support rehabilitation, quality of life, and treatment goals. Provide psychosocial support, address barriers to adherence, assist with transplant referral, and participate in ongoing QAI activities and quality reviews. Travel may be required to cover multiple facilities; role includes documentation, education, and collaboration with insurance coordinators to address coverage and transportation resources.
Required Qualifications
- Master of Social Work (MSW) degree
- State licensure as required for social work
- 2-5 years related experience
- Fresenius Medical Care experience preferred
