Job Summary
Master Social Worker (MSW) provides psychosocial services to dialysis patients and families, collaborates with interdisciplinary team to address barriers to treatment adherence, coordinates transplant referrals and education, supports staff understanding of ESRD psychosocial impact, and contributes to Quality Improvement activities. Entry-level MSW role focused on patient assessment, care planning, counseling, education, referral to community resources, and adherence support, with duties including documenting interactions, participating in team meetings, and upholding FMCNA social work policies.

- Masters in Social Work
