Job Summary
Master Social Worker (MSW) responsible for psychosocial services to dialysis patients, including assessment, care planning, counseling, and support within an interdisciplinary team. Conducts psychosocial assessments, facilitates education on treatment modalities and transplant referrals, aids in adherence and quality-of-life improvements, and documents interventions. Requires state-licensed MSW with 0–2 years of related experience; entry-level role within Fresenius Medical Care. Travel may be required between facilities. Focus on addressing barriers to rehabilitation and optimal treatment outcomes, patient education, and collaboration with the care team to support patient and family needs.
