Job Summary
Entry-level Master of Social Work (MSW) providing psychosocial services to in-center and home dialysis patients; conducts psychosocial assessments, develops care plans, and provides counseling to support rehabilitation, adjustment and quality of life. Collaborates with the interdisciplinary team, educates staff on the psychosocial impact of chronic kidney disease, facilitates transplant referrals, connects patients with community resources, and contributes to quality improvement initiatives and QAI activities. Requires knowledge of social work theory, patient education programs, and licensure requirements; supports patient education tailored to learning needs and participates in routine team discussions, discharge planning, and advocacy to promote adherence and treatment outcomes.
