Job Summary
Master Social Worker (MSW) provides psychosocial services to dialysis patients, collaborates with interdisciplinary team to assess psychosocial status, develop and implement care plans, provide counseling and education to patients and families, facilitate transplant referrals, and support quality improvement initiatives (QAI/Quality Indicators) to enhance adherence and quality of life for ESRD patients. Responsibilities include patient assessment and care planning, education tailored to patient learning needs, coordination with insurance and community resources, and participation in weekly team activities and performance reviews.
Required Qualifications
- Master's in Social Work (MSW)
- State license (as required by applicable regulations)
- 2–5 years related experience
