Job Summary
Entry-level MSW role providing psychosocial assessment, care planning, counseling, and education to dialysis patients and families; collaborates with an interdisciplinary team to address emotional, psychological, and behavioral impacts of Chronic Kidney Disease, facilitates transplant referrals, supports patient education and access to community resources, documents per MSW interaction and policy, participates in quality review meetings, and contributes to QAI activities to improve quality of life for patients.
Required Qualifications
- Masters in Social Work
- Must have state required license
- 0-2 years related experience
