Master Social Worker - MSW

Assess patients' psychosocial status, strengths, and areas of need to inform comprehensive care planning and rehabilitation goals. Provide psychosocial support, counseling, and educational interventions tailored to patients with chronic kidney disease, focusing on adherence, quality of life, and transplant readiness. Collaborate with the interdisciplinary team to facilitate referrals for community resources, vocational rehabilitation, and insurance assistance while documenting all interactions and interventions. Participate in weekly team huddles, monthly quality review meetings, and process improvement activities to address non-adherence and service recovery. Maintain updated patient resource lists and assist staff with psychosocial training as needed.
