Job Summary
Assess patients' psychosocial status and strengths to inform comprehensive care planning and rehabilitation goals. Provide psychosocial counseling, education on chronic kidney disease, and referrals to community resources such as vocational rehabilitation and home health services. Monitor patient adherence to dialysis, address quality of life concerns, and facilitate advance directive discussions with the healthcare team. Collaborate with the interdisciplinary team on transplant waitlist management, insurance coordination, and monthly quality review meetings. Conduct staff interviews, participate in weekly team huddles, and provide training on psychosocial topics. Adhere to caseload guidelines and document all interventions per facility policy.
Required Qualifications
- Masters in Social Work
- Must have state required license
- 2 – 5 years' related experience
- Travel required (if multiple facilities)
