Job Summary
Assess patients' psychosocial status and strengths to inform comprehensive care planning and rehabilitation goals. Provide psychosocial support, counseling, and education tailored to patients with Chronic Kidney Disease, including those seeking transplant, while facilitating referrals to community resources and vocational rehabilitation. Collaborate with the interdisciplinary team to address quality indicators, non-adherence, and advance directive discussions, ensuring alignment with Fresenius Kidney Care Quality Assessment and Improvement activities. This entry-level role supports the FKC commitment to patient satisfaction and quality of life through active participation in monthly quality review meetings and weekly team huddles.
Required Qualifications
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
