Master Social Worker - MSW

Provide psychosocial services to dialysis patients as part of an interdisciplinary healthcare team. Conduct patient assessments, care planning, and counseling to support rehabilitation, quality of life, and adjustment to treatment. Utilize social work theory and established programs to address barriers, educate patients and families, and coordinate referrals to community resources. Deliver education to patients considering transplant and assist with advance directive discussions. Document interactions, participate in monthly quality reviews, and contribute to ongoing quality indicators and improvement activities. This entry-level MSW role requires a Master’s in Social Work and a state license, with travel as needed to support the clinic and patient needs.
