Job Summary
Assess patients' psychosocial status and strengths to inform comprehensive care planning and identify effective interventions for rehabilitation and improved quality of life. Provide psychosocial support, counseling, and education tailored to patient learning styles, while facilitating referrals to community resources, vocational programs, and transplant services. Collaborate with the interdisciplinary team to address adherence issues, review quality indicators, and participate in monthly quality review meetings. Liaise with insurance coordinators to resolve coverage concerns and educate patients on payment options, federal, state, and commercial benefits. Adhere to caseload guidelines and state regulatory requirements, documenting all interactions and interventions.
Required Qualifications
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
