Job Summary
Assess patients' psychosocial status, strengths, and areas of need to inform care planning and improve quality of life for dialysis and home dialysis patients. Provide supportive counseling, educational goal-directed counseling for transplant seekers, and interventions for non-adherence while collaborating with the interdisciplinary team. Address insurance, financial assistance, and billing concerns; facilitate transplant referrals and manage advance directives and DNR discussions. Maintain updated patient resource lists, assist with hiring, and contribute to weekly team huddles and monthly quality review meetings. Required Qualifications
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
