Job Summary
Assess patients' psychosocial status and strengths to inform comprehensive care planning and rehabilitation goals. Provide psychosocial support, counseling, and education tailored to patients with End Stage Renal Disease, including transplant referrals and advance directive discussions. Collaborate with the interdisciplinary team to address quality indicators, non-adherence, and barriers to treatment while managing insurance and financial assistance concerns. Participate in weekly team huddles, monthly quality reviews, and staff training on psychosocial topics. Adhere to state regulatory caseload guidelines and facility policies.
Required Qualifications
- Masters in Social Work
- Must have state required license
- 2 – 5 years' related experience
- Travel required (if multiple facilities)
