Job Summary
Assess patients' psychosocial status and strengths to inform care planning with the interdisciplinary team. Provide psychosocial counseling, education on chronic kidney disease, and goal-directed support for transplant referrals. Collaborate on quality improvement activities, review quality indicators, and facilitate patient education regarding treatment modalities and rights. Serve as a liaison for insurance issues and coordinate referrals to community resources. Participate in weekly team huddles, maintain updated patient resource lists, and adhere to caseload guidelines. This entry-level role supports Fresenius Kidney Care's commitment to improving quality of life for dialysis patients through comprehensive psychosocial interventions.
Required Qualifications
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
- Must be able to lift 50 lbs
