Job Summary
Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals for in-center and home dialysis patients. Provide psychosocial support, counseling, and education on chronic kidney disease, transplant processes, and community resources to improve quality of life and treatment adherence. Collaborate with the interdisciplinary team to address non-adherence, review quality indicators, and participate in monthly quality review meetings. Liaise with insurance coordinators to resolve billing issues and facilitate access to financial assistance programs. Conduct patient interviews regarding discharge requests and advance directives, while assisting with staff hiring and providing training on psychosocial topics. Requires a New York state license (LICSW or LCSW) with 0–2 years of experience; 12 weeks of paid training provided.
Required Qualifications
- New York state licensed
- Licensed Independent Clinical Social Worker, LICSW
- Licensed Certified Social Worker, LCSW
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
- Travel required (if multiple facilities or home visits, if applicable)
