Job Summary
Assess patients' psychosocial status, strengths, and areas of need to inform care planning and rehabilitation goals. Provide psychosocial support, counseling, and education on kidney disease, transplant processes, and advance directives. Collaborate with the interdisciplinary team to address quality indicators, non-adherence, and service recovery while participating in monthly quality review meetings. Liaise with insurance coordinators regarding coverage, billing, and entitlement programs. Assist with staff hiring, maintain updated patient resource lists, and contribute to weekly team huddles. This entry-level role supports Fresenius Kidney Care's commitment to improving quality of life for dialysis patients through comprehensive psychosocial interventions and community resource referrals.
Required Qualifications
- Masters in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0 – 2 years' related experience
