Job Summary

Provide individual and group therapy using a stage-based model while utilizing safe crisis prevention techniques. Develop and revise treatment goals, document clinical progress notes within 24 hours, and complete client intake and recurrent assessments. Coordinate aftercare, discharge services, and community linkages with external providers, while assisting clients with housing, budgeting, and daily living skills. Transport clients when necessary and collaborate with interdisciplinary teams to ensure comprehensive case management. Maintain proactive communication with stakeholders, attend required supervision and training, and adhere to all privacy and regulatory guidelines.

Required Qualifications

  • Graduate Degree in psychiatry, psychology, social work, nursing, education, rehabilitation, or any graduate degree in the field of human services
  • Registered Nurse with five years' experience in the mental health field
  • Certification by the Department of Public Welfare, Office of Mental Health (OMH), as a mental health family-based worker
  • Minimum of 1 year of clinical or rehabilitation experience working with adults with severe and persistent mental illness
  • Internship and/or practicum experience conducted in a clinical inpatient or outpatient setting
  • Valid driver's license
  • Knowledge of various therapy techniques and counseling strategies applicable to serving members with persistent mental health challenges
  • Knowledge of common behavioral disorders exhibited by adults with persistent mental health challenges (specifically Bipolar, schizophrenia, schizoaffective disorder)
  • Strong written and verbal communication skills

  • Ability to make independent decisions when circumstances warrant such action
  • Strong interpersonal skills to establish and maintain effective working relationships with client, child, parent/guardian, treatment team members, and other personnel involved in treatment

  • Knowledge of and adherence to Mandated Reporter protocol
  • Ability to work collaboratively with members of the treatment team and other service providers to ensure comprehensive case management
  • Ability to use creativity appropriately to drive progress

  • Ability to provide timely and professional service to both internal requests and the requests of the people we support

  • Ability to document treatment data/results that are clinically relevant on progress notes and submit in a timely manner (within 24 hours of each encounter)
  • Ability to complete client intake paperwork/assessments per company/program guidelines
  • Ability to complete recurrent assessments within the regulated timeframes
  • Ability to participate in interdisciplinary team meetings and other miscellaneous meetings, as necessary
  • Ability to collaborate with other providers to secure appropriate aftercare/discharge services and community linkages
  • Ability to develop, implement, evaluate, and revise treatment goals and plans in collaboration with client and team members
  • Ability to assist clients in finding safe and affordable housing
  • Ability to coordinate detoxification services with external inpatient service providers and establish linkage to self-help programs
  • Ability to assist clients in budgeting and daily living skills to maintain stability in home and community settings
  • Ability to transport clients when necessary
  • Ability to complete criminal background Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Ability to pass drug testing
  • Ability to provide education verification and other credentialing based on position requirements
  • Ability to provide proof of employment history or references (if required)
  • Ability to provide proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals
