Master Medical Social Worker (MSW) - Hospice

Provide compassionate end-of-life care support to patients and families through direct counseling, case management, and coordination of hospice services. Conduct comprehensive assessments to determine eligibility for hospice benefits and develop personalized care plans addressing physical, emotional, and spiritual needs. Collaborate with interdisciplinary teams including physicians, nurses, and volunteers to ensure seamless care delivery and facilitate advance care planning discussions. Maintain accurate documentation of patient progress and communicate regularly with families regarding treatment goals and changes in condition.
