Master level social worker dialysis

Master-level social worker position focused on providing psychosocial services to dialysis patients (in-center and home dialysis when applicable) as part of an interdisciplinary team. Responsibilities include patient assessment and care planning, counseling to address psychosocial barriers to rehabilitation and quality of life, education for patients and families, facilitating transplant referrals, coordinating with insurance and community resources, and participating in quality improvement and team discussions. Requires MSW, state social work licensure, and 2–5 years of related experience; travel may be required for multiple facilities.
