Job Summary
Market Physician Executive (MPE) leads in-home multi-specialty polychronic care within a defined market, collaborating with APPs, RNs, SWs, and PharmDs to deliver evidence-based, in-home care. Responsibilities include executing clinical guidelines, improving population health outcomes, overseeing direct patient care and treatment plans, supervising frontline teams, managing care plans in Salesforce/Athena, driving cost-of-care reduction, and leading high-risk rounds. The role emphasizes clinical leadership, governance of care pathways, and population health strategies across the market, with remote work within the designated market and occasional in-home visits. Must hold an MD or DO, be board-certified or board-eligible in a specified specialty, and maintain unencumbered licenses; hospital privileges may be required.
Required Qualifications
- MD or DO degree from an accredited medical school
- BC or BE in an ACGME approved specialty (Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics)
- Active, unrestricted state medical license in each state within the market
- Willingness to obtain hospital privileges at required facilities
- Experience with kidney care and geriatrics
- Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment
- Current state medical license free of sanctions from Medicaid or Medicare
