Job Summary
Conduct telephonic, community-based, and in-home outreach to Sentara Health Plans members across the state to explain benefits, access resources, and ensure engagement. Serve as a liaison between members, community partners, healthcare providers, and internal departments to close communication gaps and strengthen community relationships. Perform required home visits to support direct, in-person engagement for members needing assistance. Utilize CRM databases and Microsoft Office Suite while applying knowledge of social determinants of health to address barriers affecting access to care. Requires a Bachelor's Degree, prior health plan product support experience, and proficiency in JIVA, QNXT, and Talroo.
Required Qualifications
- Bachelor's Degree
- Experience working with CRM Database
- Experience in customer service, public relations, outreach and advocacy
- Must have prior experience in health plan product support
- Must be proficient in Microsoft Office Suite
- Must be able to lift 50 lbs
- Home visits
- Use of personal vehicle
- Knowledge of social determinants of health
- Experience working in JIVA
- Experience working in QNXT
- Experience working in Talroo
