Job Summary
Meet and work with individuals and families to write annual service plans and create Virtual Community Resource Maps detailing jobs, housing, and transportation. Teach technology use for navigation and travel training while building community networks and supporting independent living goals. Provide monthly face-to-face meetings and virtual check-ins, document progress toward milestones, and complete service notes within deadlines. Act as a liaison between the individual, family, and technology vendors to ensure access to enabling tools. Ensure safety compliance, adhere to agency policies, and maintain a person-centered environment.
Required Qualifications
- High School Diploma/GED
- Experienced user of technology
- Must be at least 21 years of age
- Valid Tennessee Driver's License, with an F endorsement, and a clear driving history
- Must maintain liability and auto insurance
- Must maintain CPR and/or First Aid Certification and meet all DDA and agency training requirements
